The student nurse is in the clinical setting with the nursing
instructor at a Behavioral Health-Care Center. The student’s assignment is to
act in the role of a participant observer during interaction with an anxious
and withdrawn patient. The student is reminded that positive client outcomes
depend on the nurse’s ability to maintain self-awareness, respect for the
patient’s unique individuality, and client-centered interaction. (Learning

1. Identify
which theory is being utilized in this situation. Describe the specific
components of that theory.
Sullivan’s theory of social interaction and Peplau’s further
development of that theory through her concept of the therapeutic nurse–patient
relationship have influenced current nursing practice as structure and management
of the milieu (environment) is a primary role for the nurse in terms of
providing safety and protection for all clients. The nurse’s role as a
participant observer promotes therapeutic social interaction by assisting the
patient to move through the healing process. The use of social interaction
theory also promotes healthy, appropriate social interaction by the student
nurse being available to the patient should the patient choose to engage the
student nurse in conversation.
1. What
are the most influential factors that determine a patient’s response to
therapeutic interventions? How can the student nurse facilitate a positive
outcome?
The patient’s feelings and perceptions about his or her
situation are the most influential factors in determining the patient’s
outcome. Use of the therapeutic intervention, social interaction theory in this
instance, helps the patient identify and clarify his or her feelings and
perceptions through the student nurse’s availability to the patient for
engagement in therapeutic communication and establishing a therapeutic rapport
with the patient so that he or she trusts the student nurse enough to share
these feelings and perceptions.

Henry is a 34-year-old father of three children and married for
the last 10 years. He has a mortgage and other living expenses related to
maintaining a family environment. He has had difficulty finding employment
because of chronic alcoholism and marijuana use. The bank has foreclosed on his
mortgage, and he is now homeless. During the last year, Henry has become
despondent and delusional. (Learning Objective 4)
1. What
resources would be available for a homeless mentally ill client without
health-care insurance or financial resources to meet basic needs?
Henry’s mental illness will require intervention in either an
acute care or a community setting. Government-supplemented programs such as
Social Security Disability and Medicaid are available to those clients who meet
financial eligibility. Providing housing alone does not significantly alter the
prognosis of homelessness for persons with mental illness. Psychosocial
rehabilitation services, peer support, vocational training, and daily living
skill training are all important components for decreasing homelessness and
improving quality of life. In the early 1990s, the federal government
authorized a grant program to address the needs of people who are homeless and
have mental illness.

are the barriers to providing care for the homeless individual with mental
illness?
Poverty among people with mental illness is a significant
barrier to maintaining basic needs such as housing, financial resources, and
private health-care insurance.

is the coordinating role of the psychiatric nurse case manager in the discharge
planning phase of care for Henry?
The case manager works with clients on a broad range of issues,
from accessing needed medical and psychiatric services to carrying out tasks of
daily living such as using public transportation, managing money, and buying
groceries. Discharge planning from an acute care setting cannot be successful
if clients do not follow through with the established plan.
